This site collects personal information from you with your approval. By acces
sing and using wellatwork.com.au (the “Website”), you will be regarded as having consented to collection of your personal information in accordance with this privacy policy..

The personal information that you provide when using the Health Risk Appraisal (“HRA”) questionnaire in the “Work? Check!” section is collected by WLS’ agents via links to the Website as they have the proprietary software necessary to compile reports in relation to the HRA. These reports are available only to you (and nobody else) and only available during the time in which you use the HRA and the personal information provided is not stored by WLS or our agent.

Personal information may also be needed by WLS to tailor relevant information for you, as in, personalising your home page, etc. We may request that you provide personal details as part of a contest or promotion. We use this information to operate the Website and we may occasionally inform you of new features, services and products from wellatwork.com.au. In all cases, we will explicitly inform you as to the requirement and/or use of the requested information.

2. Information Collected
We collect the following information from you:

The personal information collected is used for generating personal reports, anonymous statistical analysis and/or systems administration purposes.

3. Personal Health, Credit Card Information Security and Usage
The Website uses Secure Socket Layer (SSL) technology to encrypt all data collected. Raw data from any survey(s) you take may be used anonymously to produce aggregate reports that can be used to study population health characteristics and needs for preventive health programs.

When you use your credit card for making a payment, your credit card details are processed immediately and no details are stored by us.

4. Data Quality and Security
We will take reasonable steps to ensure that the personal information WLS and HRA collect, use or disclose is accurate, complete and up-to-date. We will take reasonable steps to protect the personal information from misuse and loss due to unauthorised access, modification or disclosure. We will also take reasonable steps to destroy or permanently de-identify personal information if it is no longer needed.

5. Editing your personal information
This site gives users the option for updating, changing and modifying information previously provided. Visitors can change their username and password. Once login is complete, look for the link to My details.

6. E-mail addresses
We record your e-mail address so we can send you a user name and password and if you request for any information in future. We will not disclose your e-mail address without your consent.
